A fee of $40 will be charged at/after the walk if a minimum of $50 is not raised. If a team as a whole raises enough to cover everyone's fee, no one will be charged.
The undersigned agrees to indemnify and hold harmless the Uveitis/
Ocular Inflammatory Disease (OID) Support Group (USG), Ocular
Immunology and Uveitis Foundation, Essex County Department
of Parks , Recreation and Cultural Affairs , the Massachusetts
Eye Research and Surgery Institution (MERSI), and any other
organization, sponsor, or individual involved in the 2024 Walk for Vision
from all cost, expense, and liability arising out of my and/or my
child’s participation in this event to benefit Uveitis and OID research.
I do hereby waive all claims for damage or loss to me or my child’s
person or property which may be caused by an act, or failure to act,
by all the USG, Essex County Department of Parks, Recreation and
Cultural Affairs, OIUF, MERSI, and all other sponsors, organizations,
departments involved in the Walk for Vision, its officers, volunteers,
or employees arising directly or indirectly from my and/or my child’s
participation in this event; and I hereby assume liability for myself and/
or my child for any loss, damage, or other liability from such event.
I grant full permission for organizers to use photos, videos, film,
or any other record of this event in which I and/or my child may
